PERMISSION TO TREAT YOUR DOG AND
RELEASE OF LIABILITY

As the owner of your dog(s), you understand that Always Home, LLC, will exercise due care to protect the health and safety of your dog while in their care. In the event that your dog becomes ill, or injured, you will authorize Always Home, LLC to obtain medical treatment in the event of an illness or accident for my pets. I give the attending veterinarian permission to start medical treatment. You will understand that you are financially responsible for any costs or vet care for the injury or illness.

In the event that the medical expenses exceed (client to give amount) you request that Always Home, LLC contact you before any further treatment is performed. You agree to reimburse Always Home, LLC for any and all expenses incurred for the medical treatment of your dog.

You hereby waive and release Always Home, LLC , from any and all liability of any nature of injury, death or loss of my dog resulting from the negligence . If your dog would cause injury to another dog or person while at Always Home, LLC you agree to indemnify and subrogate for Always Home, LLC, from any action which may be brought against it, and for any defense, settlement, or judgment entered against it. .
